29 CFR  1910.95(i)(3): Employees were not given the opportunity to select their hearing protectors from a variety of suitable hearing protectors provided by the employer:   a. An employee operating the Dover machine was exposed to a noise dose of 103.6%, or an equivalent dBA of 90.2 dBA which was capable of causing  permanent hearing loss. The sampling was performed for 453 minutes during one shift on December 5, 2024. The employer did not provide employees  with a variety of suitable hearing protectors.   b. An employee operating Machine 26 was exposed to a noise dose of 82.5%, or a equivalent dBA of 88.6 dBA which was capable of causing permanent hearing loss. The sampling was performed for 465 minutes during one shift on December 5, 2024. The employer did not provide employees  with a variety of suitable hearing protectors.   c. An employee operating Machine 501 was exposed to a noise dose of 121.1%, or a equivalent dBA of 91.3 dBA which was capable of causing permanent hearing loss. The sampling was performed for 451 minutes during one shift on December 5, 2024. The employer did not provide employees  with a variety of suitable hearing protectors.   d. An employee operating The Blue Bird and Symtec 2 was exposed to a noise dose of 91%, or a equivalent dBA of 89.3 dBA which was capable of causing permanent hearing loss. The sampling was performed for 460 minutes during one shift on December 5, 2024. The employer did not provide employees  with a variety of suitable hearing protectors.
