29 CFR 1910.178(n)(4): Loads that obstructed forward view were not moved with the load trailing: a) Job site, 3100 East Carson Street, Pittsburgh, PA: On or about December 2, 2024, during production activities, a load was driven forward when it obstructed the drivers view resulting in a serious workplace accident when an employee on foot was struck by the forklift/its' load.

29 CFR 1910.178(l)(4)(i): Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator has the knowledge and skills needed to operate the powered industrial truck safely: a) Job site, 3100 East Carson Street, Pittsburgh, PA: On or about December 2, 2024, during production activities, the employer did not ensure that employees were provided refresher forklift training. Deficiencies noted during the inspection included, but were not limited to, driving forward with obstructed views, loading products on forklift forks that were raised approximately 3 feet in the air, standing in front of a running forklift to load products onto it, driving around with forks raised in the air and not always wearing a seat belt while operating a forklift.
