Employees utilizing the Metaltech Scaffolding Tower to clean ductwork weekly were not trained in the care, inspection, and use of the scaffolding to identify any visible defects or hazards associated with the equipment.

See 29 CFR 1910.27(a) On or about December 3, 2024, at the establishment located at 697 Main Avenue in Adell, Wisconsin; the employer neglected to inspect the Metaltech Scaffolding Tower before each work shift, exposing employees to fall hazards exceeding 10 feet. 29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects. When the falling objects are too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer shall place such potential falling objects away from the edge of the surface from which they could fall and shall secure those materials as necessary to prevent their falling. See 29 CFR 1910.27(a) On or about December 3, 2024, at the establishment located at 697 Main Avenue in Adell, Wisconsin; employees were not protected from falling objects while working near the scaffold by the installation of toe boards, screens or other protection designed to catch or deflect objects dislodged on upper levels of the scaffold.
