29 CFR 1910.28(b)(15): Except as provided elsewhere in this section or by other subparts of 29 CFR part 1910, the employer must ensure each employee on a walking-working surface 4 feet (1.2 m) or more above a lower level is protected from falling by guardrail systems; safety net systems; or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems: On or about April 2, 2025, at a facility located at 248 East Chicago Road in Wautoma, WI; an employee stepped over the guardrail system of a work platform to stand on the support structure of the super sack shifter in the Line 4 fill room. The employee was exposed to a fall of approximately 51 inches either between the sifter equipment and the ground, or off the sides of the sifter's structure to the ground.

29 CFR 1910.147(c)(5)(ii): Lockout devices and tagout devices shall be singularly identified: On May 14, 2025, at a facility located at 248 East Chicago Road in Wautoma, WI; an employee with their lock on the group lockbox, which was used to contain keys for the group lockout of the Spee-Dee hopper and auger filler equipment during changeover, was not singularly identified for the employee that placed the lock.

29 CFR 1910.147(d)(6): Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. On May 14, 2025, at a facility located at 248 East Chicago Road in Wautoma, WI; prior to initiating a changeover operations for the Spee-Dee hopper and auger filler equipment in the Line 4 fill room, the authorized employee did not verify that the augers' motors had been effectively disconnected from the electrical power source and could not be restarted during the course of the servicing task.
