On or about December 10, 2024, at 211 S Genesee Street in Merrill, Wisconsin, employees were exposed to a potential fall hazard working on a roof 16 feet above surface level without the warning line system installed at least six feet from the southern roof edge. The warning line ends were tied to the side rails of the roof access ladder.

Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about December 9, 2024, at 211 S Genesee Street in Merrill, Wisconsin, employees were exposed to a fall hazard working on a roof between a warning line system and the roof edge 16 feet above surface level without required combination fall protection equipment. The Durable Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1780379, citation number 01, item number 01a, and was affirmed as a final order on November 8, 2024, with respect to a workplace located at 203 Central Bridge Street, Wausau, WI 54401.
