29 CFR 1910.253(b)(2)(ii): Inside of buildings, cylinders shall be stored in a well-protected, well-ventilated, dry location, at least 20 (6.1 m) feet from highly combustible materials such as oil or excelsior. Cylinders should be stored in definitely assigned places away from elevators, stairs, or gangways. Assigned storage spaces shall be located where cylinders will not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons. Cylinders shall not be kept in unventilated enclosures such as lockers and cupboards. Engineering Shop: On/about December 13, 2024, two oxygen and two carbon dioxide gas cylinders were not well protected and could be subjected to being knocked over or damaged. Employees were exposed to struck by and asphyxiation hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use: On/about December 13, 2024, two eye wash stations were not available for immediate use due to the water valve being shut off. Employees were potentially exposed to corrosive chemicals in the following locations: a. Outside Chiller/Cooler Building: Employee use ChemTex 3560-OT (pH 11-14), ChemTex 6269 (pH 11-14), ChemTex 9160 (pH <1), and ChemTex AA-4215 (pH 1-4) to treat water. b. Near Ice Maker, Main Kitchen: Employees use Betco Symplicity Sanibet Multi-Range (pH 6-9) to clean. The Avi Resort & Casino was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.151(c), which was contained in OSHA inspection number 1560981, citation number 01, item number001 and was affirmed as a final order on 12/16/2021, with respect to a workplace located at 10000 Aha Macav Parkway in Laughlin, NV 89029.
