29 CFR 1926.20(b)(2): A program of frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons was not in use: a) Jobsite, 1 Windsor Way, Pittsburgh, PA: On or about December 16, 2024, during roofing work, the employer did not ensure that a system of hazard identification and correction was implemented at the job site. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection where fall hazards of up to approximately 30 feet existed, allowing 2 people to simultaneously climb a ladder, ground persons working below others on roof with no head protection and ladder being set up crooked.
