29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:   On or about June 23, 2025, at a residential new build job site, located at 2571 Wintergreen Lane, Ravenna OH 44266, the employer failed to train employees in the recognition of potential fall hazards and effective means of prevention and protection from such. Employees were exposed to a fall from height hazard of 12 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about June 23, 2025, at a residential new build job site, located at 2571 Wintergreen Lane, Ravenna OH 44266, the employer failed to ensure that a portable, multi-purpose ladder that was being used to access the roof was placed at least three feet about the upper landing surface.  Employees were exposed to a fall from height hazard of 12 feet.

29 CFR  1926.1060(a)(1): The employer did not provide a training program for each employee using ladders as necessary, enabling each employee to recognize hazards related to ladders and the procedures to be followed to minimize these hazards:  On or about June 23, 2025, at a residential new build job site, located at 2571 Wintergreen Lane, Ravenna OH 44266, the employer failed to ensure employees who were using a portable, multi-purpose ladder to access a roof, were trained in the recognition of potential fall hazards and ladder safety and in an effective means of prevention and protection from such.  Employees were exposed to a fall from height hazard of 12 feet.

29 CFR  1926.501(b)(13):  Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about June 23, 2025, at a residential new build job site, located at 2571 Wintergreen Lane, Ravenna OH 44266, the employer failed to ensure employees, who were working on a residential roof of approximately 12 feet, were protected from a fall from height hazards in that employees were not wearing a personal fall protection harness nor were there any other fall protections in place.  Ever Ivan Garcia Hernandez was previously cited for a violation of this occupational safety and health standard or its equivalent Standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1793765, citation number 1, item number 1 and was affirmed as a final order on February 12, 2025 , with respect to a workplace located at 1 Windsor Way, Pittsburgh, PA 15237.
