Conneautville Soybean Crushing, LLC, outdoor silo area - On or about 12/17/24, and at times prior thereto, employees were climbing the fixed silo ladder with muddy shoes creating a potential slip and fall hazard. The walking surface leading to the ladder contains mud. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Conneautville Soybean Crushing, LLC ; Production area - On or about 12/17/24, and at times prior to, an electrical knockout box was hung from the ceiling by conduit. The conduit was not restrained, and the electrical knockout box was not mounted. The knockout box was part of a 3 phase motor for a press. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

The items stored in front of the panel, included, but were not limited to, plastic trays, aerosol can, piece of metal and a paper bag.
