29 CFR 1926.417(a): Controls that were to be deactivated during the course of work on energized or de-energized equipment or circuits were not tagged: a) Facility - On or about December 18, 2024, Circuit Breaker 26 on panel (MP1) (277 Volt, 20 amp circuit) was not de-energized and not tagged by employees working from a mobile scaffold (Genie - GS-1930) with live wires to install a new outlet exposing the employees to electrocution hazards.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) Facility - On or about December 18, 2024, employees working on a mobile scaffold (Genie - GS-1930) were not trained by a person qualified in the subject matter to recognize hazards associated with the type of scaffold being used to include employees being exposed to electrocution hazards due to working with live wires to install a new outlet (277 Volt, 20 amp circuit).
