On or about December 11, 2024 - Laborer employees did not use appropriate flammable resistant clothing while burning combustible materials such as wood pallets and cardboard boxes accelerated with liquid flammable then lite with a handheld flare.

On or about December 11, 2024 - The portable fire extinguisher located in the outside weather storage area was not mounted, located and identified so that they were readily accessible. The fire extinguisher was between a portable ladder and the wall.   b) Inside workshop - 421 North 2nd Street, West Newton PA. On or about December 30, 2024 - The portable fire extinguisher located on the work bench by the welding area was not mounted, located and identified so that they were readily accessible.

On or about December 11, 2024 - The portable fire extinguisher located in the outside weather storage area was not subjected to an annual maintenance check. The fire extinguisher had no inspection record attached.   b) Inside workshop - 421 North 2nd Street, West Newton PA. On or about December 30, 2024 - The portable fire extinguisher mounted near the front door was not subjected to an annual maintenance check. The record indicated the last maintenance was performed in 2022.   c) Inside workshop - 421 North 2nd Street, West Newton PA. On or about December 30, 2024 - The portable fire extinguisher mounted near the rear door was not subjected to an annual maintenance check. The record indicated the last maintenance was damaged.  d) Inside workshop - 421 North 2nd Street, West Newton PA. On or about December 30, 2024 - The portable fire extinguisher located on the work bench by the welding area was not subjected to an annual maintenance check.  The fire extinguisher had no inspection record attached.

Employees were not specifically provided information and training on hazard category meaning the division of criteria within each hazard class, e.g., oral acute toxicity and flammable liquids include four hazard categories.
