(a) Worksite: On or about 01-02-2025, an employee was exposed to fall and struck-by type hazards from riding on an unattached and unguarded platform in an elevated position driven by a powered industrial truck.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) Worksite: On or about 01-02-2025, employees were exposed to fall hazards while performing framing activities from an elevated work surface, approximately 12 feet 8 inches above the lower level, without any means of fall protection.
