29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use: On or about December 28, 2024, at a facility located at 2491 Old Highway 51 in Kronenwetter, WI; the employer did not provide suitable facilities for quick drenching or flushing of the eyes and body for immediate emergency use when an employee was exposed to injurious corrosive material in the form of ammonia vapors. The emergency eyewash/shower located in the Auxiliary Boiler room did not provide tepid temperatures.

29 CFR 1910.303(g)(1)(i)(A): The depth of the working space in the direction of access to live parts may not be less than indicated in Table S-1. Distances shall be measured from the live parts if they are exposed or from the enclosure front or opening if they are enclosed: On or about December 28, 2024, at a facility located at 2491 Old Highway 51 in Kronenwetter, WI; the depth of the working space in front of an electrical panel for the Heat Trace system in the Lifting Bay. Portions of dismantled scaffolding, metal tubing, pallets, batteries, and sheeting did not allow for 3 feet of access to the electrical panel.
