29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l): a) On or about January 8, 2025, the employer failed to ensure an employee who operated a powered industrial truck was properly trained, evaluated, and certified to perform the tasks in a safe manner, thereby exposing employees to struck-by hazards. Wells Plumbing and Heating Supplies Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.178(l)(1)(i), which was contained in OSHA inspection number 1732644, citation number 1, item number 4 and was affirmed as a final order on June 20, 2024, with respect to a workplace located at 4101 West 123rd Street, Alsip, IL 60803.
