On or about January 6, 2024, at an establishment located at 451 Joannes Avenue in Green Bay, Wisconsin, the employer did not ensure nameplates were maintained in a legible condition on the Crown Double Reach #DJ170805 electric powered forklift exposing employees to potential forklift hazards.
