29 CFR 1910.134(c)(2)(ii): The employer did not establish and implement those elements of a written program necessary to ensure that any employee using a respirator voluntarily was medically able to use that respirator, and that the respirator was cleaned, stored, and maintained so that its use does not present a health hazard to the user: On or about January 14, 2025, the employer did not provide a medical evaluation to determine an employee's ability to wear a half face tight-fitting negative pressure respirator before the employee was allowed to voluntarily use it while performing welding operations.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): On or about January 14, 2025, employees were exposed to hazardous energy sources and the employer did not ensure energy isolation procedures included the minimum required elements: a. Lockout procedures for the Amada brake press model HFB 2204 did not identify gravity or hydraulic energy sources and include specific instructions for controlling and verifying the energy sources had been dissipated and isolated. b. Lockout procedures for the Mazak Super Turbo-X612 MkII laser did not identify energy sources from the use of compressed nitrogen gas and include specific instructions for controlling and verifying the energy source(s) had been dissipated and isolated. c. Lockout procedures for the Ingersoll Rand Air Compressor Machine did not include specific instructions for dissipating and verifying that pneumatic energy had been isolated and controlled.
