29 CFR 1910.178(m)(10): Load backrest extension(s) were not used on a powered industrial truck equipment to minimize the hazard of rearward falling materials: Employees were exposed to struck-by hazards. Employees engaged in material handing activities using walk behind powered industrial trucks in the back storage area located at 14955 W. 151st Street, Olathe, Kansas were exposed to struck-by hazards in that the load backrest did not prevent unwrapped pallet loads of product from falling rearward when placed in the upper storage racks.

29 CFR 1910.178(m)(4): The employer did not prohibit arms and/or legs from being placed between the uprights of the mast and/or outside the running lines of the truck: Employees engaged in operating the Crown walk behind powered industrial trucks at the grocery store located at 14955 W. 151st Street, Olathe, Kansas were exposed to struck-by hazards due to the mast guard not preventing the operator's body parts from accessing the mast.
