29 CFR  1926.451(c)(2)(iv):Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:  a) Worksite was located at 425 2nd Avenue, Spring Lake, NJ 07762  A worker was exposed to fall hazards of approximately two-stories to ground as they stood on a wooden pallet being used as a scaffold platform while working. The platform was being supported by a Gradall 534C Material Handler. The pallet was not secured to the machine.  Violation was observed on or about January 14, 2025  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

Violation was observed on or about January 15, 2025  The FS Alvarado Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1598796 , citation number 1, item number 1 and was affirmed as a final order on 03/29/2023, with respect to a workplace located at 17 Washington St, Spring Lake, NJ 07762.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
