29 CFR 1910.29(b)(3):29 CFR 1910.29(b)(3): The employer did not ensure that guardrail systems are capable of withstanding, without failure, a force of at least 200 pounds applied in a downward or outward direction within 2 inches of the top edge, at any point along the top rail. (a) Extrusion Line #5, Mezzanine; On or about January 16, 2025, and at times prior, the employer exposed employees to fall hazards, in that a panel of the guardrail system was not secured with ground bolts and was incapable of withstanding 200 pounds of lateral (outward) force.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) Extrusion Line #5, SPC Machine; On or about December 29, 2024, and at times prior, the employer exposed employees to caught-in hazards, in that there was no guarding at the SPC Extrusion Machine in-rolling nip point between Emboss Roll and Rubber roll that would prevent employees being exposed to moving parts at the point of operation during the operating cycle.
