29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about January 13, 2025, in the Cut & Wrap Room, employees were using vertical harp cutters to process Edam cheese and had their fingers and hands exposed to caught-in hazard likely to result in serious injuries including amputation. Vertical harp cutters were not effectively guarded to prevent access to the cutting wires and push plate during movement. The operator's controls were not synchronous and concurrent during hazardous cycle and the helper was placing their hands within inches of the cutting wires to prevent cut cheese pieces from falling off the plate.

On or about May 13, 2025, on the Roll Stock machine line, employees were standing at their workstations by the unguarded rotating shaft with its end protruding approximately 2.5 in. (being more than half of its diameter) and exposing employees to caught-on entanglement hazard.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.    a) On or about May 13, 2025, employees working in and around the engineering room/office were exposed to electrical and fire hazards due to a relocatable power tap (power strip) being affixed to a wall and modified for direct wiring to a nearby wall-mounted electrical panel to serve in lieu of a branch-circuit receptacle.    b) On or about May 13, 2025, employees working in the IT room were exposed to electrical and fire hazards due to multiple relocatable power taps (power strips) being affixed to a metal wall framing track and connected in series (daisy-chained) to each other in lieu of branch-circuit receptacles.

An unqualified person wired new connections in the electrical cabinet of the MultiVac packaging machine and then conducted live voltage testing on 240V AC circuits. The required training was not provided in the skills and techniques necessary to distinguish exposed live parts from other parts of the electrical equipment and the safety-related work practices, including selection and use of necessary electrical protective equipment, intended to provide safety relative to electrical hazards.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   The employer failed to establish an energy control lockout/tagout program in the workplace to control hazardous energy sources (e.g. electric, pneumatic, etc.) on production equipment prior to assigning unauthorized personnel to perform maintenance and servicing tasks such as, but not limited to, setup, adjustments, teardown, cleaning and sanitation:   a) On or about January 10, 2025, operators were assigned to perform setup on the energized RollStock Ri-200 packaging machine (Cut & Wrap line). Operators had their upper extremities exposed to pinch point caught-in hazards of moving conveyor parts likely to result in serious injuries. An unexpected start-up of the conveyor (indexing) at the non-forming film section during an adjustment task caused a recordable injury because lockout was not performed.   b) On or about January 22, 2025, operators were assigned to disassemble the energized World Cup Rotary Cup Sealer (Line A in Crumble Room) for cleaning and sanitation. Operators had their upper extremities exposed to caught-in and struck-by hazard of moving parts such as the rotary table and push pistons likely to result in serious injuries when machine was not locked out. Previously, an unexpected startup of the equipment during an adjustment task caused a recordable injury because lockout was not conducted.
