29 CFR  1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  (a) On January 15th, 2025, and at times prior, at 4245 12th Avenue North Fargo, North Dakota, employees were exposed to fall hazards up to 6 feet 8 inches while working on elevated platforms to attach moldings to Ferry Rotospeed machines without proper guardrails or overhead fall protection systems in place.

(k) On January 15th, 2025 and at times prior, at 4245 12th Avenue North Fargo, North Dakota, guarding was not provided to protect employees from hazards created by the rotating chuck on the SHARP Lathe.  This condition exposed employees to struck-by hazards by contacting the rotating chuck.
