29 CFR  1910.147(c)(4)(ii):  The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  a)  At an establishment located at 3100 East Frontage Road in Kaukauna, Wisconsin in the Assembly Area; the energy control procedures did not not clearly and specifically outline the purpose and scope of the procedure for the installation of the guide tubes, die blocks, sleeves, and the changing of the blade for the Marken Model 475A and the Marken Model 550 w/ D.M. hose cutter equipment.  b)   At an establishment located at 3100 East Frontage Road in Kaukauna, Wisconsin in the Assembly Area; the energy control procedures did not identify all applicable energy sources including but not limited to pneumatic power on the Marken Model 475A and Marken Model 550 w/ D.M. hose cutter equipment.

29 CFR  1910.147(c)(6)(i):  The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about October 1, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area; the employer had not accomplished a periodic inspection of the lockout / tagout procedures and the authorized employees for the Marken Model 550 w/DM and Marken Model 475A hose cutters which were put into service October 1, 2021.

29 CFR  1910.147(c)(7)(i)(A):  Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.  On or about the week of October 17, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area; the employer did not provide training to recognize all applicable hazardous energy sources as well as the means and methods to accomplish proper energy isolating steps exposing employees to accidental or inadvertent restart of the equipment while performing servicing and maintenance work including the installation of the guide tubes, die blocks, sleeves, or changing of the blade.

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)  During the week of October 17, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area; employees were exposed to machine hazards associated with moving parts when they engaged in blade changes activities on the Marken Model 475A.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to perform blade changes.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  (b)  On or about October 31, 2022 and thereafter, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area; employees were exposed to machine hazards associated with moving parts when they engaged in setup activities on the Model:  550 w/D.M., S/N:  3078.  The employer did not ensure that employees implemented energy control application steps or equivalent protective measures prior to allowing them to perform setup activities.  The machine was not deenergized prior to the employee engaging in the servicing / maintenance activity.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.

On or about November 1, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin; the employer did not provide training and evaluations of the forklift operator(s) to ensure the operator could operate safely.

29 CFR  1910.178(q)(7):  Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.  On or about November 1, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area; the green Clark Electric Forklift TMX 15s was not inspected on a daily basis to ensure no adverse conditions were present which could affect the safety of the equipment exposing employees to struck by or tipping hazards.

During the week of October 17, 2022, at an establishment located at 3100 East Frontage Road, in Kaukauna, Wisconsin in the Assembly Area;  quad metallic electrical pedant boxes were suspended used to plug in various equipment and not surface mounted exposing employees to electrical shock hazards.
