29 CFR 1910.147(d)(1):Preparation for shutdown. Before an authorized or affected employee turns off a machine or equipment, the authorized employee shall have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy. On or about December 2, 2024, the employer failed to ensure in preparation for machine shutdown authorized employees, working as outside personnel (contractors), performing service and maintenance activities by replacing the Metallus, Inc., Harrison Steel Processing Plant, Finishing Department, C Building, #2 Magnaglo Machine table rolls had the knowledge of the type and magnitude of the hazardous gravitational energy of the #2 Magnaglo Machine's Easy Down Arm component(s), knew the hazards of the gravitational energy of the #2 Magnaglo Machine's Easy Down Arm components, and had the knowledge of the methods and means to control the gravitational energy of the #2 Magnaglo Machine's Easy Down Arm components before the machine was turned off and prior to conducting the service and maintenance activities.

29 CFR 1910.147(d)(6):Verification of isolation. Prior to starting work on machines or equipment that have been locked out or tagged out, the authorized employee shall verify that isolation and deenergization of the machine or equipment have been accomplished. On or about December 2, 2024, the employer failed to ensure authorized employees, working as outside personnel (contractors), performing service and maintenance activities by replacing the Metallus, Inc., Harrison Steel Processing Plant, Finishing Department, C Building, #2 Magnaglo Machine table rolls verified that isolation and deenergization of the hazardous gravitational energy of the #2 Magnaglo Machine's Easy Down Arm component(s) had been accomplished prior to starting the replacement work on the #2 Magnaglo Machine table rolls.
