On or about January 22, 2025, the employer failed to ensure that the LP Toyota Forklift Model: 8FGU32, Serial Number: 12836, and LP Toyota Forklift Model: 42-5FG25, Serial Number: 405PG25-15941, with front-end fork attachments other than the factory installed attachments identified the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered, thereby, exposing affected employees to tip over hazards.

The LP Toyota, Model 42-5FG25, Serial Number 405PG25-15941 was allowed to be operated while having a leaking hydraulic cylinder and inoperable horn, thereby exposing affected employee(s) to struck-by hazards.

Employee(s) were exposed to crushing injuries while using the #1 Fuchs 340 Material handler when the structural components failed during normal operations and resulted in a catastrophic equipment failure.
