29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. a) 697 Doremus Ave. Newark, NJ: An employee was injured while working from a Liebherr LTM 1400-7.1 Crane truck. The employees was hospitalized and the employer failed to notify the Occupational Safety and Health Administration in the required 24 hours. Occured on or about 01/14/2025. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
