29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  a) On or about  January 23, 2025 at 5230 Citrus leaf Blvd Lot 292 Winter garden Fl 34787, Lot 292: the employer allowed employees to use extension ladders to gain access to the roof and the rails of the ladders did not extend at least three feet above the roof landing to provide safe access exposing employees to a fall hazard of up to 10 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  (a) On or about  January 23, 2025 at 5230 Citrus leaf Blvd Lot 292 Winter garden Fl 34787, Lot 292: employees were exposed to fall hazards while installing siding on a roof at heights of 11 feet, in that, fall protection was not used.   RMA State Renovations Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1510397, citation number 1, item number 2 and was affirmed as a final order on 10-04-2023, with respect to a workplace located at 167 Linlake Cove, Oviedo FL.
