29 CFR  1926.451(b)(5)(i):The end of a platform 10 feet or less in length, extended over its support more than 12 inches.  a.) At the worksite, located at 727 Steel Dr Apollo Beach, Fl 33572; On or about 7/31/25, the employer exposed employees to struck by and fall hazards of approximately 8ft  in that pump jack scaffold platform extended over its support more than 12 inches while conducting siding operations.

29 CFR  1926.452(j)(2):Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  a.) At the worksite, located at 727 Steel Dr Apollo Beach, Fl 33572; On or about 7/31/25, the employer exposed employees to struck by and fall hazards of approximately 8ft  in that pump jack scaffold poles were not secured to the house structure with bottom bracing systems while conducting siding operations.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  a.) At the worksite, located at 727 Steel Dr Apollo Beach, Fl 33572; On or about 7/31/25, the employer exposed employees to fall hazards, of approximately 6 ft, in that, a Werner 6ft A-frame ladder was not used in accordance with the manufacturers instructions as it was propped in the closed position, leaning against the pump jack scaffold for access while conducting siding operations.
