29 CFR 1910.106(e)(2)(ii)(b)(2): More than 120 gallons of Category 2, 3, or 4 flammable or combustible liquids in containers were located outside of inside storage room(s) or storage cabinet(s): At the painting area in the manufacturing building located at 202 Boeh lane, Wathena, Kansas, on or about January 28, 2025, employees engaged in painting activities were exposed to fire hazards in that seven 55-gallon drums filled with #24LH-3000 Red Oxide, category 3 flammable liquid, were being stored in the painting area.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At 202 Boeh lane, Wathena, Kansas, on or about January 28, 2025, employee (s) were exposed to amputation hazards in the following instances: a). In rail shop at the manufacturing building, WT drill press was not guarded. b). In rail shop at manufacturing building, Wells horizontal bandsaw was missing guarding exposing approximately 4 inches of unused portion of the blade to the right of the point of operation. c). In main fabrication shop, 1150 drill press was not guarded.
