29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about January 29, 2025, employees were exposed to fall hazards, while performing gutter installation work on a residential home located at 2804 Inglewood Ave NW, Canton Ohio 44718; during the installation of gutters the employer did not ensure portable ladders used to gain access/egress to the roof (working surface) extended 3 feet above the upper landing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    On or about January 29, 2025, employees were exposed to fall hazards, while installing gutters on a residential home located at 2804 Inglewood Ave NW, Canton Ohio 44718; the employer did not ensure employees working on a residential roof were protected from a fall of approximately 18 feet by utilizing a fall protection system.  Silver Leaf Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1585593, citation number 02, item number 01and was affirmed as a final order on (6/21/2022), with respect to a workplace located at 6742 Johnson Street SE, Massillon Ohio 44646.
