29 CFR  1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:  On or about December 2, 2024, the employer failed to ensure the hazardous energy control (lockout/tagout) procedure for the Harrison Steel Plant, Finishing Department, C Building, #2 Magnaglo Machine clearly and specifically outlined the specific procedural steps for blocking and securing the gravitational energy source of the machine's Easy Down Arm components [per 1910.147(c)(4)(ii)(B)], and included specific requirements for testing the Easy Down Arms to determine and verify the effectiveness of the gravitational energy control measures [per 1910.147(c)(4)(ii)(D)], for employees and/or outside personnel (contractors) engaged in service and maintenance activities, such as but not limited to, replacement of the #2 Magnaglo Machine table rolls.

29 CFR  1910.147(d):Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  On or about December 2, 2024, the employer failed to ensure employees and/or outside personnel (contractors) performing service and maintenance activities by replacing the Harrison Steel Processing Plant, Finishing Department, C Building, #2 Magnaglo Machine table rolls applied hazardous energy control; ensuring all the potentially hazardous gravitational energy of the #2 Magnaglo Machine's Easy Down Arm component(s) was relieved, restrained, and/or otherwise rendered safe [per 1910.147(d)(5)(i)], and verifying de-energization and isolation of the Easy Down Arms gravitational energy had been accomplished [per 1910.147(d)(6)], prior to replacing the table rolls.       Metallus, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d) which was contained in OSHA inspection number 1678340, citation number 1, item number 1 c, and was affirmed as a final order on 12/22/2023, with respect to a workplace located at 45011 Faircrest Street, SW, Canton, Ohio.

29 CFR  1910.147(c)(6)(i):The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  On or about December 2, 2024, the employer failed to conduct a periodic inspection of the developed and documented energy control (lockout/tagout) procedure for the Harrison Steel Plant, Finishing Department, C Building, #2 Magnaglo Machine to correct any inadequacies in the procedure [per 1910.147(c)(6)(i)(B)], and certify the periodic inspection had been performed with the date of the inspection, employees included in  inspection, and the person performing the inspection [per 1910.147(c)(6)(i)(D)], to ensure the procedure and the requirements of the standard were being followed by employees and/or outside personnel (contractors) engaged in service and maintenance activities, such as but not limited to, replacement of the #2 Magnaglo Machine table rolls.    Metallus, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(6)(i) which was contained in OSHA inspection number 1678340, citation number 1, item number 2, and was affirmed as a final order on 12/22/2023, with respect to a workplace located at 45011 Faircrest Street, SW, Canton, Ohio.
