Flowery Branch, GA 30542: On or about 1/29/2025, the employer exposed employees to head injuries while working inside of an unprotected 7-foot, 4-inch deep excavation in that hard hats were not worn where there were visible overhead hazards present to include but not limited to loose rocks, earth, and construction materials at the edge.
