On or about January 29, 2025, employee(s) were engaged in residential reroofing activities at 10715 Cimarron Street, Papillion, Nebraska. Employee(s) were using a ladder to access and egress the roof. The ladder was placed on the rake of the roof resulting in the two rails of the ladder not being supported equally as required by this sub-part.

The employer is failing to protect employee(s) from fall hazards. Employee(s) were engaged in residential re-roofing activities on or about January 29, 2025, at 10715 Cimarron Street, Papillion, Nebraska. Employee(s) were exposed to fall hazards greater than 11 feet when roofing a commercial building without using any form of fall protection. NDG Contractors dba NDG Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1667734, citation number 2, item number 1 and was affirmed as a final order on 6-16-2023, with respect to a workplace located at 10723 Cimarron Street, Papillion, Nebraska.
