29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly:  Employees engaged in making pesticide products activities were exposed to burns, fire hazards in that fire extinguishers were not inspected monthly at the following instances: a) Building 1 by the front door - Employer did not inspect the fire extinguisher in December 2024. b) Building 1 in the back of the building - Employer did not inspect the fire extinguisher in December 2024. c) Building 6 - Employer did not inspect the fire extinguisher in December 2024. d) Fuel Tank area - Employer did not inspect the fire extinguisher in February, March, April, May, June, July and in December 2024.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by rotating parts and flying chips:   Employees engaged in machining activities at the Evolution Mag Drill Press in the Maintenance Shop Building #6 were exposed to struck-by hazards in that machine guarding was not provided. The rotating drill chuck was not guarded, employees were not provided protection from flying chips and debris.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:   Employees performing servicing and maintenance operations were exposed to burns, electrical shock hazards in that the following Electric Disconnects and Breakers missing labels to identify what they service: a) Building #1, the Double D Square safety switch box was not labeled. b) Building #3, the 240VAC 1PH Service Disconnect box was not label. c) Building #9, the 480V Double D safety switch box was not labeled. d) Outdoor Poll - Fuel Tank area, the Eaton Cutler-Hammer Heavy Duty Safety Switch box was not labeled.  e) Building #11, the circuit breaker slots #1 and #5 were not labeled.
