29 CFR 1910.37(b)(2):Each exit was not clearly visible and marked by a sign reading "Exit": a. Engine Room 3/PDC - On or about February 5, 2025, the exit door designated as the secondary evacuation route by the posted evacuation diagram was not marked with an "Exit" sign, exposing employees to ammonia in the event or a release. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.145(c)(3): Safety instruction sign(s) were not used where there was a need for general instructions and suggestions relative to safety measure(s): a. Engine Room 3/PDC - On or about February 5, 2025, the king valve was obstructed by materials being stored in front of the valve preventing access in the event of an emergency. Safety instruction signs were not utilized to notify employees of the importance of having clear access to the king valve to shut down the ammonia system. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
