29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. At an establishment located at N6637 North Rolling Meadows Drive, Fond Du Lac, Wisconsin; (a) On or about February 3, 2025, in the Press Area, the DGM Press 2, Tower 3 had an unguarded nip point between the green and silver rollers due to the guard being removed exposing employees to caught in between hazards. (b) On or about February 3, 2025, in the Press Area, the DGM Press 1 and 2, employees were exposed to caught between hazards due to the unguarded slitter blades and perforated blades.

On or about the week of January 20, 2025, at an establishment located at N6637 North Rolling Meadows Drive, Fond Du Lac, Wisconsin; by DGM Press Side 1, a single metallic outlet box was not surfaced mounted exposing employees to electrical shock hazards.

29 CFR 1910.303(g)(2)(i): Except as�elsewhere required�or permitted�by this�standard, live�parts of�electric equipment�operating at�50 volts�or more�shall be�guarded against�accidental contact by�use of�approved cabinets�or other�forms of�approved enclosures�or by�any of�the following�means: In December 2024 and thereafter, at an establishment located at N6637 North Rolling Meadows Drive, Fond Du Lac, Wisconsin; on the DGM Press 1, electrical panel cover on the back side of the folder had been removed in order to reset the switches. The exposed electrical connection points by the Cabur Switching Power Supply were energized exposing employees to electrical shock hazards.
