29 CFR 1910.147(d)(4)(i):Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: On or about January 28, 2025, employees are engaged in commercial manufacturing activities, located at 1 Frances Street, Morrill, NE 69358. Employee are exposed to laceration hazards to include amputation when conducting unjamming activities on equipment that was not locked-out. Instances include but are not limited to: i. Indumak Model Number: MU-320, Serial Number: 812595, Side B. ii. Wexxar, Model: WF-20T, Serial Number: 4743 (and the four other Case Former Machines).
