29 CFR  1926.102(a)(1):The employer did not ensure that each affected employee was using eye or face protection when exposed to eye or face hazards from flying particles.  At a new residential worksite located at 7300 block NW Pampas KC MO -- employees engaged in framing duties using hammers, pneumatic nail guns and other various small hand tools were not wearing any eye protection. This exposed employees to getting shrapnel and/or debris in their eyes causing injury. This was documented on or about 2/6/2025.

29 CFR  1926.405(g)(2)(iii): Flexible cords were not used only in continuous lengths without splice or tap:   At a new residential worksite located at 7300 block NW Pampas KC MO -- An orange extension cord was spliced together and had black electrical tape around the spliced section. This exposed employees to possible electrical burns and/or shocks. This was documented on or about 2/6/2025.

29 CFR  1926.405(g)(2)(iv):  Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws :  At a commercial worksite located at 7300 block NW Pampas, KC MO -- A black flex cord for the portable air compressor had damaged strain relief exposing the inner wires. There was also damaged strain relief at the female end of an orange extension cord. These exposed employees to possible electrical burns and/or shocks. These was documented on or about 2/6/2025.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At a new residential worksite located at 7300 block NW Pampas, KC MO -- employees engaged in framing duties were using a wood job-made ladder to access the roof. The ladder did not extend 3 feet above the upper landing surface. This exposed employees to fall hazards of 12' to ground level. This was documented on or about 2/6/2025.
