29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the jobsite located at 4231 Clary Blvd, Kansas City, MO 64130, employees were exposed to caught-between hazards due to inadequate guarding at the cutting dies on the Peerless E57 paper plate-making machines. As part of their regular duties, employees are required to clear jams in the cutting dies of the machines. However, the guards on the dies are secured by screws that become loose during normal operation. These loose screws undermine the effectiveness of the guards, allowing them to be easily displaced and granting employees access to hazardous areas of the machinery.
