29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the jobsite located at 4231 Clary Blvd, Kansas City, MO 64130, employees were exposed to caught-between hazards due to inadequate guarding at the forming dies on the Peerless E57 paper plate-making machines. As part of their regular duties, employees are required to clear jams in the forming dies of these machines. Currently, the guards on the dies are secured by screws that frequently become loose during normal operation allowing guards to be easily displaced. Additionally, the spacing of the guards is approximately 2.5 to 3 inches apart, which allows employees to place their hands or limbs between the guards. The combination of loose screws and the inadequate spacing of the guards undermines their effectiveness, thereby granting employees access to hazardous areas of the machinery.
