29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the employer's energy control procedures required by this standard:    (a)Midwest Manufacturing dba Dakota Panel at 1651 Culvert Street, Rapid City, SD 57701: On or about and at times prior to February 10, 2025, the employer did not ensure specific energy control procedures to deenergize the Biesse Bander 3 Equipment - Infeed Saw Area were utilized prior to operators entering the machine area/danger zone. This condition exposed employees to crushing, amputation, shock, and air embolisms while accessing the equipment.
