OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a) On or about February 10, 2025, employees were exposed to struck-by hazards in that industrial storage racks were visibly damaged with bent and twisted beams and columns, and damaged or missing anchor bolts in locations included, but are not limited to locations, Aisle 10, locations 1019 & 1020; Aisle 11, location 1109; Aisle 12, locations 1208, 1209, 1213, 1214, 1217, 1218 & 1221; Aisle 13, locations 1317 & 1318; Aisle 14, location 1410; Aisle 15, location 1511; and Aisle 16, location 1607. Wood products including, but not limited to, hollow and solid core wood doors, were stored on multiple levels of the racking systems. (b) On or about February 10, 2025, employees were exposed to struck-by hazards where industrial storage racks were missing weight limits throughout the warehouse. Wood products including, but not limited to, solid and hollow core doors, were stored on multiple levels of the racking systems.
