Facility Loading Dock, 1155 Allegheny Avenue, Oakmont, PA 15139: On or about February 6, 2025, the employer did not ensure boxes of styrofoam and aluminum extrusions were properly stacked in a manner that would prevent against unintentional sliding and collapse.
