29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: a) In the production area, on or about 02/07/2025, the employer exposed employees to fall hazards, in that, employees worked from aerial lifts at heights of approximately 20 feet without using a personal fall arrest or restraint system while working in the basket of an aerial lift.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) For the rebar process and other galvanizing processes at the facility, on or about 02/07/2025 and at times prior to, the employer exposed employees to burn hazards, in that, personal protective equipment to protect employees' bodies, arms and portions of neck and back/top of head was not provided and required to be used by employees. Employees were exposed to splash hazards from molten zinc at temperatures of around 800 degrees Fahrenheit.
