29 CFR  1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  At the temporary job site located at 15326/15328 Kristina St. Wichita, Kansas, employees engaged in residential roofing activities using pneumatic nail guns were exposed to struck-by hazards in that eye protection was not utilized.  Carlos Soria was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1752982, citation number 1, item number 1 and was affirmed as a final order on 09-16-2024, with respect to a workplace located at 2269-2271 North 159th Ct. East, Wichita, Kansas  Carlos Soria was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1644307, citation number 1, item number 2 and was affirmed as a final order on 06-13-2023, with respect to a workplace located at 630 Sweet Water Cir. 29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   At the temporary job site located at 15326/15328 Kristina St. Wichita, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the eave of the roof to the ground below in that fall protection was not utilized.  Carlos Soria was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1753023, citation number 1, item number 1 and was affirmed as a final order on 09-16-2024, with respect to a workplace located at 2287-2289 North 159th Ct. East, Wichita, KS 67228.  Carlos Soria was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1753004, citation number 1, item number 1,  and was affirmed as a final order on 09-16-2024, with respect to a workplace located at Lot 18 - 12438 West Fennec St., Maize, KS 67101. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  At the job site located at 15326/15328 Kristina St. Wichita, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 11 feet from the eave of the residence to the ground below in that the ladder used to access the roof was not extended at least 3 feet above the eave or was not secured to the structure.
