On or about February 11, 2025, employees engaged in residential roofing activities, located at 12009 S 110 Ave Cir., Papillion, NE 68046. Employee(s) were exposed to fall hazards when working on a roof that was measured in excess of 6-feet. Employee(s) where conducting residential roofing operations without having been trained on: the hazards associated with working at elevation and the essential aspects of fall protection equipment (inspection, wear and use).

On or about February 11, 2025, employees where engaged in residential roofing activities, located at 12009 S 110 Ave Cir., Papillion, NE 68046. Employee(s) were exposed to fall hazards. Employee(s) where working on a roof that was measured in excess of 6-feet without any form of fall protection. Gaines Framing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1487340, citation number 1, item number 1 and was affirmed as a final order on October 2, 2020, with respect to a workplace located at 12908 S 51st St., Bellevue, NE 68123.
