(b) Production floor: On or about 2/18/2025, the employer did not ensure that the WC 222, SN: S03663-18-YK Yasnac LX3 CNC machine was locked and tagged out when a rear panel guard was removed, exposing employees to ingoing nip points of a pulley and belt.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. (a) Production floor: On or about 2/18/2025, the employer did not ensure that the WC 321 Webb lathe machine was equipped with an adequate guarding method that would prevent employees from reaching around the guard and placing a body part into the danger zone.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and/or sparks. (a) Production floor: On or about 2/18/2025, the employer did not ensure that the WC 312, SN: 124DN454649, CNC machine was equipped with a guarding method to protect employees from the rotating part on the left side of the machine. The Jade Engineered Plastics, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1639423, citation number 1, item number 1 and was affirmed as a final order on 06-29-2023, with respect to a workplace located at 15 New Industrial Way Warren, RI 02885.
