This was observed on or about February 14, 2025, at 204 E Roberts St., Grand Island, Nebraska. The employer is failing to ensure that process hazard analysis for protecting ammonia equipment from struck-by hazards were addressed. The sight glass on the East High Pressure Receiver (EHPR) in the East engine room being struck was not addressed on the most recent PHA completed in 2022.

29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment: Employee(s) are exposed to chemical hazards associated with the anhydrous ammonia refrigeration system. This was observed on or about February 14, 2025, at 204 E Roberts St., Grand Island, Nebraska. The employer is failing to ensure that ammonia equipment was inspected at the intervals that they designated. A pipe carrying anhydrous ammonia on the roof released 10,000 lbs. of the chemical when a portion of the pipe that was under insulation corroded to the point of failure. The piping under the insulation on the roof was last inspected in 2017, and the required schedule for this inspection is every 5 years.
