29 CFR 1910.178(o)(1): Load(s) on powered industrial truck(s) were not stable or safely arranged. a) Worksite in the vicinity Bay 166 - On or about 2/19/2025, employees were exposed to struck by hazards from operating a pallet jack with an unstable load of chairs stacked approximately 8-feet high which resulted in a load of chairs falling and striking another operator. b) Worksite in the vicinity of Bay 164 - On or about 2/19/2025, employees were exposed to struck by hazards from operating a pallet jack with an unstable load of chairs stacked approximately 8-feet high placed to the right of the operators load-backrest, resting solely on the right tine only. The employee was operating the pallet jack with one hand and holding the stack of chairs with the other hand.
