29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On January 18th, 2025, and at times prior, at 1600 North 48th Street Grand Forks, North Dakota, the employer did not ensure that the point of operation on the Pirahna Ironworker serial no. P3-141 located in the shop area, was adequately guarded at the sheer blade. This condition exposed employees to amputation and laceration hazards.

(a) On January 4th, 2025, at 1600 North 48th Street Grand Forks, North Dakota, the employer did not report an amputation of an employee's middle finger while operating the Piranha iron works machine until 9 days after the incident.
