29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated; the person certifying that the evaluation has been performed; the date(s) of the hazard assessment; and, which identifies the document as a certification of hazard assessment. On or about February 21, 2025, the employer failed to create and implement a written Personal Protective Equipment (PPE) hazard assessment for employees operating ELCV lines used to combine the rubber product and Duel Line 12. Operations include but is not limited to setting up and running product through the paint booth and drills on Line 12 and sliding and adjusting products on the ECLV Line.
